Fill out the Passport Application Form below

Take it to the nearest immigration office to get the finger prints done;

Please use Fingerprint Barcode Form Below

Bring it to the Embassy with

• copy of your birth certificate,
• copy of your passport to be renewed,
• copy of your national identification ID
• 2 passport size photos

Please fill out COUNTER SIGNATURE Form Below
